How it works

  1. ALS patients and/or caregivers who have attended an ALS clinic in the the past 2 years fill out a survey. It takes about 8 minutes. They can fill out a survey every 6 months if a condition at their clinic changes significantly. An ALS clinic in this context does not have to be a multidisciplinary clinic. It can be a clinic with only a neurologist or only an ALS-specializing practitioner.

  2. No personal information is collected from survey respondents. There’s an option for respondents to provide their email if they are ok with possibly being contacted by the survey administrator. The survey admin (email: mapping4nonprofits@aol.com) is the only person who has access to the email address EVER.

  3. Each survey is screened before publishing to make sure no inappropriate content gets published. If a respondent provides an inappropriate comment, the objectionable content is removed and the remainder of the survey is published.

  4. Within a few days (typically 1 day), completed surveys are published. Every survey, in its entirety (minus respondent’s email when provided), can be reviewed in the Advisor app. This app (website) is open to the public and is designed for desktops.

  5. Clinics can respond to any survey comment by having their clinic administrator email the survey admin at mapping4nonprofits@aol.com. No log-in is needed. The clinic response will appear next to survey comment in the Advisor app within a few days.

  6. A clinic administrator can request an aggregate report on their own clinic by emailing the survey admin at mapping4nonprofits@aol.com.
